Instructor Information
To participate as an instructor in the Recreation Unlimited Adaptive Ski Program, please follow the procedures found on this page.  The following two forms must be completed.  The forms may be viewed and printed by clicking the links below.  You may print these instructions by printing the Instructor Cover Letter.

1. Instructor Enrollment Form
2. Adult or Minor Liability Form

Fill out the forms completely and mail to the following address:

You may also bring your completed forms to the Dry Land Clinic on:

Dec 4, 2007   6:00-8:30 P.M.     1720 Westgate, Rm 119, Suite D.
(INSTRUCTORS MUST ARRIVE AT 6:00 P.M.)

Instructors, please note that ALL PAPERWORK and DUES must be turned in by the Dry Land Clinic!

*** ALL FORMS AND FEES ARE DUE BY THE DRY LAND CLINIC ***

Please be aware of the changes on the enrollment form under the time preferences area.  Following are some instructions on how to complete the form:

1. Dues are $35.00 per year, $50.00 in raffle ticket sales, or any level of Corporate Sponsorship.
2. Check either Saturdays or Sundays.
3. Check No Preference if you do not care whether you ski on Saturday, Sunday, morning, or afternoon.
