Student Information
To participate in the Recreation Unlimited Adaptive Ski Program, please follow the procedures found on this page.  The following three forms must be completed.  The forms may be viewed and printed by clicking the links below.  You may print these instructions by printing the Student Cover Letter.

1. Student Enrollment Form
2. Medical Release Form
3. Adult or Minor Liability Form

Fill out the forms completely and mail (with lesson fees) to the following address:

You may also bring your completed forms to the Dry Land Clinic on:

Dec 4, 2007   7:00-8:30 P.M.     1720 Westgate, Rm 119, Suite D.
(STUDENTS PLEASE DO NOT ARRIVE BEFORE 7:00 P.M.)

Please note lessons are assigned on a first come, first served basis.  Your forms will be stamped the day they arrive in our office.  All paperwork, dues, lesson fees, and medical releases must be received before a final date stamp will be placed.

*** ALL FORMS AND FEES ARE DUE BY THE DRY LAND CLINIC ***

Please be aware of the changes on the enrollment form under the time preferences area.  Following are some instructions on how to complete the form:

1. Dues are $35.00 per year, $50.00 in raffle ticket sales, or any level of Corporate Sponsorship.
2. Put in the number of lessons you want (1 lesson = 2 hours, 6 lessons maximum).
3. Check either half day or full day lessons.
4. Put in the amount it will cost for the lessons.
5. Check either Saturdays or Sundays (Sundays are more available).
6. Check either Mornings or Afternoons.
7. Check No Preference if you do not care whether you ski on Saturday, Sunday, morning, or afternoon.
